Create a Check-in Form
Building Your Form
To create a check-in form, go to Checks > Forms and click + New Form. Give it a clear name like “Weekly Progress Check” or “Measurement Day” — you can have multiple forms for different purposes.
Adding Fields
The form builder lets you drag and drop fields into your form. Available field types include:
- Number — for weight, measurements, or any numeric value (supports metric and imperial units)
- Text — open-ended questions like “How did this week go?”
- Photo upload — request front, side, or back progress photos
- Scale (1–10) — for subjective ratings like mood, energy, stress, or adherence
- Yes/No — simple binary questions like “Did you hit your step goal?”
- Multiple choice — let clients select from predefined options
Required vs Optional Fields
Mark fields as required or optional depending on what you need. Required fields must be completed before the client can submit. Keep required fields to a minimum — shorter forms get higher completion rates.
Preview and Test
Click Preview to see exactly what your client will see in the app. The preview works on desktop and simulates the mobile layout so you can check spacing and flow.
💡 Tip: Start with a simple form (weight, photos, mood) and add fields later. Overloading clients with too many questions on day one leads to drop-off.
Assigning the Form
Once saved, you can assign the form to individual clients or groups. Each client can only have one active check-in form at a time.
